Job Title: RN Case Manager
Location: Sacramento, CA
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 16:30)
Description:
- Must have acute hospital experience in CM and discharge planning
- Must have prior travel experience
- Certification is not required.
- Must be willing to work in weekend rotation
Required:
-1 yr. minimum recent experience with discharge planning as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital setting
- Need to have acute INPATIENT case manager experience.
- BLS
- InterQual
- EPIC
- Allscripts/Careport
- Utilization Review is a plus
- ACM or CCM Certification is a plus

Pay: $55.00 - $60.00 per hour
Expected hours: 40 per week
